Sexuell übertragbare Erkrankungen (STI-STD) sind jene Krankheiten, die hauptsächlich durch Geschlechtsverkehr übertragen werden können. Sie können von Viren, Bakterien, Pilzen, Protozoen ausgelöst werden. Zu Ihnen zählen unter anderem HIV, Syphilis, Virushepatitiden, Chlamydien und Tripper (Gonorrhoe). Für einige Infektionen besteht eine gesetzliche Meldepflicht (anonym, Hepatitis ABC namentlich).
Es sind einige Impfungen verfügbar wie zum Beispiel Hepatitis A+B und HPV (Humane Papillomaviren), um sich vor einer Infektion zu schützen. Vor anderen Infektionen kann man sich durch Kondome oder wie bei HIV auch durch eine PREP schützen. Safer Sex ist wichtig, schützt aber nicht zu 100 % vor STIs. Erkrankungen wie Syphilis und Tripper können auch beim konsequenten Gebrauch von Kondomen übertragen werden, selbst wenn Kondome das Übertragungsrisiko senken.
Deshalb ist es wichtig, sich regelmäßig auf STI untersuchen zu lassen. Denn nicht immer wird eine Infektion bemerkt. Eine frühe Diagnose ermöglicht eine zeitnahe Therapie und verhindert, dass die Infektion an andere weitergeben wird. Die meisten STI lassen sich durch Medikamente heilen. Für andere wie HIV existieren Medikamente um die Infektion unter Kontrolle zu halten, eine Krankheitsprogression zu verhindern und auch Übertragung auf andere zu minimieren.
Medizinische Fachgesellschaft in Deutschland ist die Deutsche STI-Gesellschaft (DSTIG - Gründung 1912). Des Weiteren gibt es die Deutsche Gesellschaft für Infektiologie (DGI - Gründung 1973) und die Deutsche Arbeitsgemeinschaft niedergelassener Ärzte in der Versorgung HIV-Infizierter e.V. (DAGNÄ - Gründung 1990).

STIs are illnesses which are transmitted mainly sexually. They are caused by bacteria, viruses, fungi or protozoon. To the STI belong among others HIV, Lues (Syphilis), viral Hepatitis, Chlamydia and Gonorrhea. Some of these infections have to be assigned to the German health institution (partly anonymous, Hepatitis ABC with name).
Some vaccinations are existing as for hepatitis A+B or HPV. Condoms can reduce the risk of acquiring STIs. For HIV there is also PREP-therapy available to avoid transmission. Safe sex is important, nevertheless safe sex can´t prevent all of those diseases (like chlamydia or syphilis).
It is important to get testet regularly for STI, especially if you have alternating sexual partners. You might not recognize every infection. An early diagnosis gives you the chance for an efficient treatment and helps avoiding transmission to others. The majority of all STI can be cured. For others like HIV there are treatments to suppress viral replication, avoid HIV related illnesses and HIV-transmission.
Medical societies in Germany are the German STI Society (DSTIG - founded 1912), the German Society for Infectious Diseases (DGI - founded 1973) and the Deutsche Arbeitsgemeinschaft niedergelassener Ärzte in der Versorgung HIV-Infizierter e.V. (dagnä – founded 1990).

STI: Routine Test

Wir haben Zeit einen passenden Termin zu finden:
A – Neu-Patienten in unserer Praxis vereinbaren bitte telefonisch einen Termin einem unserer Ärzte (bitte bringen Sie Ihren Impfpass mit). Der Arzt bietet Ihnen danach schnellstmöglich den daraus folgenden Besprechungstermin an. Bitte haben Sie grundsätzlich Verständnis dafür, wenn wir bei hohem Arbeitsaufkommen nicht immer diese Routine anbieten können.
B – Uns bekannte Patienten können auch online einen Termin direkt zum Labor buchen. Jede Woche werden dazu neue Termine online frei gegeben. Wie ist der weitere Ablauf ?
1. Vereinbarter Labortermin Blutabnahme HIV, Lues, Hepatitis C
2. Arzttermin – Ergebnisse von 1. + evtl. Abstriche in Mund, Anus (bei Analverkehr), Urinabgabe (2-4 Std. vorher kein Wasserlassen)
3. Telefonat über weitere Ergebnisse
HIV-Test: Testmöglichkeiten und Zeitfenster
>6 Wochen nach Risiko: HIV-Antikörper-Schnelltest (Ergebnis nach mind. 30 min - Selbstzahlerleistung)
Schnell bezieht sich auf das Tempo des Ergebnisses NICHT auf den zeitlichen Abstand zur Risikosituation. Wir benutzen einen Schnelltest der 4.Generation, der eine HIV-Infektion zu 99% bei >6 Wochen ausschließen kann.
6 – 2 Wochen nach Risiko: HIV-Antikörper-Standard-Test über Speziallabor (Ergebnis nach 1-3 Tagen)
Diese Test sind sensitiver als der HIV-Schnelltest.
<2 Wochen nach Risiko: HI-Virus Direktnachweis (Ergebnis in 1-3 Tagen)
Dieser sehr teure Test wird nur bei wenigen ausgewählten Situationen eingesetzt oder kann als Selbstzahlerleistung angefordert werden.
HIV-Antikörper-Selbsttest zu Hause ?
Erste HIV-Selbstteste sind seit 2018 in Deutschland zugelassen und für Patienten in Apotheken verfügbar. Die Zuverlässigkeit und das diagnostische Zeitfenster ist abhängig von den Herstellerangaben. Als Orientierung beträgt das notwendige Zeitfenster wahrscheinlich häufig >12 Wochen nach Risikokontakt. Nähere Information finden Sie auch auf der Seite des Paul-Ehrlich-Institutes. Im August 2018 startete das Pilotprojekt SAM Mein Heimtest in Bayern. Es wird nicht nur HIV sondern u.a. auch Syphillis, Hepatitis C, Chlamydien und Gonokken untersucht. So haben dann auch Patienten im ländlichen Raum Zugang zu regelmäßigen STI-Tests.

A – new patients in our clinic: please call us to schedule an appointment with one of our doctors. (please bring your vaccination-records/history). The doctor will offer you the next available talk about results. Please understand in times of much work with sick patients, that we can´t always offer this routine.
B. Known patients from our office have online access to a couple of appointments directly in our lab. Each week there will be new opened timeframes. We will run a blood-check with HIV, Syphilis, Hepatitis C). We will schedule smears-Test, where necessary, in the mouth, anus (after anal intercourse) and a urine sample (with no urination 2-4 hours before). Another talk or appointment will be arranged for the results.
HIV test: testing options and time window
> 6 weeks after risk: a rapid HIV-antibody-test with a result after 30 min is available for direct payers.
(Fast refers to the result of the test NOT on the time interval to the risk situation. We use a 4th generation quick-test, which detects up to 99% of HIV-antibodies >6 weeks after risk)
6-2 weeks after risk: a standard HIV-antibody-standard-test via special laboratory with result in 1-3 days
(this test is much more sensitive then the quick-test)
< 2 weeks after risk: direct detecting of HI-virus with result in 1-3 days
(only available in a few selected medical situations or for direct payers, very expensive)
HIV-self-testing at home ?
The first HIV self-tests have been approved in Germany 2018 and are available for patients in pharmacies. The reliability and the diagnostic time window depends on the manufacturer's information. The necessary window of opportunity is most likely > 12 weeks after risk contact. Additional informations under Paul-Ehrlich-Institutes. In August 2018, the pilot project SAM Home Test launched in Bavaria. In addition to HIV are tested syphilis, hepatitis C, chlamydia and gonorrhea as well. This will give patients from the country side access to routine STI testing.

Hepatitis B: drei Impfungen innerhalb 6 Monaten (bedingt Kassenleistung)
Humanes Papillom Virus (>Condylome >Gebärmutterhalskrebs): drei Impfungen innerhalb 6 Monaten (keine Kassenleistung für Erwachsene)

There are existing the following vaccinations for STI:
Hepatitis A: two shots in 6-24 months (sometimes payed by the insurance)
Hepatitis B: three shots in 6 months (sometimes payed by the insurance)
Humane Papilloma Virus (can cause >condyloma >cervical cancer): three shots in 6 months (not payed by insurance for adults)

At the appointment with the doctor for an informal interview, we will check your vaccination records and explain you the possible vaccinations and the insurance costs / reimbursements; please bring your vaccination record. Additional vaccinations can be done, like travel-recommendations including yellow-fever.

HIV-PREP
Die PREP ist eine medikamentöse Behandlung um das Risiko einer HIV Infektion zu verringern. Sie ist neben dem Gebrauch von Kondomen ein effektiver Schutz vor HIV. Für die PREP wird das aus der HIV Therapie bekannte Medikament Truvada ® verwendet. Es besteht aus den beiden Wirkstoffen Tenofovirdisoproxil und Emtricitabin.
Vor Beginn mit der PREP ist es wichtig eine bereits bestehende HIV Infektion auszuschließen, um eine ungenügende Therapie und damit auch Resistenzen zu vermeiden. Um die Verträglichkeit der PREP zu gewährleisten ist es wichtig vor und während der Therapie ausgewählte Blutwerte zu kontrollieren. Eine dreimonatige Testung auf HIV und andere sexuell übertragbare Infektionen ist obligat. Zurzeit wir die PREP noch nicht von den Krankenkassen übernommen – es gibt Pläne zur Einführung als Kassenleistung.

Wie ist der Ablauf ?

3. Beim Start erneut HIV-Test + Nieren-/Leberwerte
4. Laborkontrolle nach 3-4 Wochen: ohne Hinweis auf Nebenwirkungen verschreiben wir das Medikament für drei Monate und planen dazu die Routine-Kontrollen alle drei Monate.

PREP is a medical treatment to reduce the risk of HIV transmission. Beside condoms PREP is an efficient way to avoid HIV infections. For PREP we use the well known drug Truvada ® from the HIV treatment. It contains two active substances (Tenovovirdisoproxil and Emtricitabine).
Before beginning the PREP treatment it is important to detect an unknown HIV infection to avoid an insufficient treatment and also resistances of the virus. To monitor the tolerability is it mandatory to check some of your health aspects before starting and during the therapy. A check of your STI-status every three months is as well important. There is no reimbursement from the insurances – there is discussion about taking it into their catalogues.

1. We start with an informal consultation with one of our doctors.
2. A blood sample 6 weeks before starting is mandatory (HIV Test etc. - please inform us if you've ever had Hepatitis C)
3. At the beginning of the therapy, we will perform another HIV Test and check your base results for any possible upcoming side effects (kidney / liver levels).
4. Laboratory control 3-4 weeks after starting: without side effects prescriptions for three months can be done in combination with routine every three months.
